If you don’t have AADHAR CARD still you are eligible to get subsidy for LPG you need to submit FORM NO.3 to Your Bank OR you can also submit FORM NO.4 to LPG Distributor. 

You are advised to fill FORM NO 4 to LPG Distributor if You Submit FORM NO 3 to bank, Bank Will Pass It To LPG Distributor for Approval so it will best if You don’t Have AADHAR CARD  submit Form No.4 to Your LPG Distributor

This is advise to LPG Consumer of all Government Oil Company, that if You have Submit Aadhar Card To Your LPG Dealer you also need to submit your aadhar card to your bank along with following Form. Fill all details in form and submit so to your Bank as soon as possible.

FORM : Change of Name

Some Time LPG Consumer Need To Change Their Name due to Death of Father/mother. At that time Consumer Must Need to Submit following Affidavit to Gas Dealer along with Death Certificate and Pedhinama.
