This is advise to LPG Consumer of all Government Oil Company, that if You have Submit Aadhar Card To Your LPG Dealer you also need to submit your aadhar card to your bank along with following Form. Fill all details in form and submit so to your Bank as soon as possible.

FORM : Change of Name

Some Time LPG Consumer Need To Change Their Name due to Death of Father/mother. At that time Consumer Must Need to Submit following Affidavit to Gas Dealer along with Death Certificate and Pedhinama. Consumer also need to submit KYC from to Dealer. you can Print format of Form for Change of Name. You can Contact us for any other advise. 

FORM : Loss of Subscription Voucher (SV)

Subscription Voucher is the most important document for the LPG consumer. you need Subscription Voucher to do Change of Name , when you need additional cylinder  and for other task also, if you have lost your  Subscription Voucher (SV) you may take print of below format for Subscription Voucher (SV) and fill details. you must need to affiliated for this document.  after doing affidavit this document will your Subscription Voucher (SV)

Now You Must Need To Submit Your KYC form To Your GAS DEALER. it may Possible you Don't have Time For So,Don't Worry KYC form as below in JPG file you may Take Print Out of This and Send to Your GAS DEALER. You may also Post so to Your Gas Dealer.
